That survey is pretty comprehensive.
The issue- you don't collect email addresses at any point in the survey to "choose a winner".
With Survey Monkey you can do this with a fill in the blank field as a last question. Just add it in. You'll get a csv of all information and can do a rand sort to pull the winner. As it stands, the survey is long and the promise of enter to win is negated.
