I would say that an OSHA investigation that says they died in an avalanche and lists the fines associated to the workplace is far from an industry investigation with the intent to educate....

Didnt say there was no investigation, just saying that it is not timely, they don't release many details and the information is hard to find. in aviation and wildland firefighting there is an industry database and required investigation....
